Cape Abilities Residential Services is a comprehensive, varied program which ensures that individuals with disabilities can choose the living situation or setting that best meets their needs to enhance their quality of life.
This program enable our participants to live as independently as possible, while still receiving the care and support they need and the security and comfort of knowing they have a life-long home.

Residential Services
Supervised Living
Supervised Living provides support to adults living in Cape Abilities operated homes, located in desirable and aesthetically pleasing community locations. In 24 hour supported environments, staff are present whenever individuals are at home. Staff work with individuals so they may meet their specific goals in areas such as increasing activities of daily living (ADL) skills, funds management and personal growth, community inclusion and relationship building.Staff are provided with training specific to the unique needs of the individuals served.

Individuals have a developmental/intellectual disability and
are eligible for services through the Department of
Developmental Services.

ABI/MFP Supervised Living
ABI/MFP Supervised Living provides services to adults living with Acquired Brain Injury (ABI) and other disabilities qualifying for the Money Follows the Person (MFP) waiver in Cape Abilities operated homes located throughout Cape Cod. Individuals with ABI are transitioned to live outside of skilled nursing facilities and other long-stay hospital settings in homes designed and built to maximize accessibility and independence. Individuals can enjoy the comforts and atmosphere of a private home setting and supports necessary to facilitate community integration, personal growth and enriched quality of life. All staff are provided with training specific to the unique needs of the individuals served.

Individuals who qualify under the ABI/MFP Waiver programs

Statewide Head Injury Program (SHIP) Residential Supports
provides services to adults living with Traumatic Brain Injury (TBI) in Cape Abilities operated community homes. Individuals can enjoy the comforts and atmosphere of a private home setting and supports necessary to facilitate community integration, personal growth and enriched quality of life. Staff is provided with training specific to the unique needs of the individuals served.

Individuals who have been diagnosed with an external Traumatic Brain Injury

Community Based

24 hours per day, 7 days per week.

Community Living/In Home Supports
offers customized assistance to people in their home and community based upon the individual’s specific needs. Supports may be in the form of providing assistance with household budgeting, making and keeping medical appointments and assisting the individual with meaningful integration into their local community.
>> Eligibility:
Individual must have a developmental/intellectual disability (including Prader-Willi syndrome and Autism) and are eligible for services through the Department of Developmental Services

Individually tailored – less than 24 hours per day, up to 7 days per week.

Adult Family Care (AFC)
is an option for Mass Health members with chronic health care needs. Individuals in AFC need help with daily living skills but wish to continue living an active lifestyle in the community. The AFC program allows people to enjoy the comforts and atmosphere of a private home with assistance from a paid family or non-family caregiver. The AFC program provides nursing case management to support the family as well as the individual. The registered nurse and case manager schedule regular home visits to monitor the placement, as well as any necessary training the AFC caregiver may need. Cape Abilities also provides 24-hour emergency support.
>> Eligibility:
Individual must be 16 years old or older, receive Mass Health insurance, either Standard or Common Health, be unable to live alone, require daily assistance in at least several of these areas; bathing, dressing, toileting, eating, transferring, and mobility within or outside of the home.

Shared Living
This placement service allows people to enjoy the comforts and atmosphere of a private home with assistance from paid caregivers who provide the opportunity to live in the community as a member of a family. Providers are trained and matched with the individual in their care to ensure stability, safety, health, and socialization. The program provides nursing care management to support the family as well as the individual. The registered nurse and case manager schedule regular home visits to monitor the placement, as well as any necessary training the family may need. Cape Abilities also provides 24-hour emergency support.

Individual must have a developmental/intellectual disability and be eligible for The Department of Developmental (DDS) services or be eligible under the Acquired Brain Injury/Money Follows the Person waivers or the Statewide Head Injury Program and must qualify for DDS funding

Individually tailored – 24 hours per day, 7 days per week.
